3. A release form, a complete patient history form, a prescription order form,
a original written doctors prescription mailed to us or have your doctor fax your prescription to us to complete your order. If the forms have been previously submitted, then just the order form is required.

If you wish to fax or mail your order you can download and print the
appropriate forms by clicking on the ‘Forms’ button at the top of this screen.
The order form generated by our shopping cart can be used instead of the hand written one.
